Script Order Form (for credit cards)

1. Find scripts you wish to order on the Scripts page. Make a note of each script number, title, and number of pages.
2. Complete the form below (you can TAB from one field to another, but do not press Return).
3. Click "submit" once at the bottom of the form.
4. After you submit this form, you'll be given a link to where you can pay for the scripts.

NOTE: some of the newer scripts are available to GLPA members for FREE as digital files (doc or pdf). If you want a script in digital format, do NOT order it here. Simply send an email to Geoff Holt: gholt@madison.k12.wi.us

Enter the total number of pages.

1. The copying cost is 10 cents per page.
2. Postage and handling is $1.50 for orders from 1 to 25 pages, $2.00 for 26-50 pages,
   and add 50 cents for each additional 50 pages or fraction thereof.
3. Round UP to the nearest dollar.
4. Enter the dollar amount here:  $ and make a note of the cost. Be ready to enter this number in the On-Line Store.
